Privacy of Clinical Information Policy

At the Child Development Network we may be collecting a good deal of information about your child and family. Initial information is collected in the form of questionnaires and copies of any reports you may provide. Additional information is collected during visits, assessments and other clinical activities. We may receive further information from other professionals (e.g. school guidance officers).

This note is to inform you regarding what happens to this information.

  1. This information is available to other members of the Professional Group working within the Child Development Network. If you do not want this to occur, please let the professional know with whom you will be working.
  2. As is the custom with medical referrals, summary letters or reports are usually sent to your referring General Practitioner, and it is our policy to provide you with a copy of all information we send to the General Practitioner.
  3. We do not automatically send copies of information to schools or any other professionals involved. If you would like us to do this, please let us know.
  4. The information about your child and family contained in the Child Development Network files will not be used for teaching or research purposes without your consent. Furthermore it will not be made available to any professionals outside of the service without your consent.
